Physical Therapist (Home Health)

Conduct segments of treatment programs to restore and improve physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and perform therapy interventions using appropriate protocols. Supervise assigned assistants and ensure all clinical documentation is comprehensive and accurate within established deadlines. Maintain valid licensure in Washington and Oregon, hold a Doctor of Physical Therapy degree, and possess current CPR certification and a valid driver's license. This role is based in The Dalles, Oregon, at Adventist Health Columbia Gorge, a 49-bed hospital and home health provider serving the Columbia River Gorge community since 1901.
