Physical Therapist Home Health

Provide physical therapy services in patients' homes across Southern California counties under a therapist-owned home health therapy referral network. Responsibilities include evaluating patients in their homes, developing and updating individualized care plans, implementing skilled PT interventions to support mobility, safety, strength, balance, and functional independence, assessing home environments for equipment or safety modifications, and documenting patient progress accurately within payer, agency, and regulatory requirements. You will coordinate with referring agencies, physicians, patients, families, PTAs, and other care participants, and you will manage documentation using a streamlined digital workflow. This role emphasizes flexible scheduling, autonomy over caseload and service areas, and contracting opportunities with the referral network. Requirements include an active California PT license, reliable transportation, professional liability insurance, a commitment to high-quality documentation, and the ability to work as an independent contractor. Expected compensation reflects visit volume and service area with a potential gross annual range of $153,000–$208,000; sign-on and referral bonuses are available.
