Provide physical therapy services in patients' homes in Southern California as a flexible independent contractor. Evaluate patients in their homes and provide skilled PT interventions to support mobility, safety, strength, balance, functional independence, and quality of life. Develop, update, and implement care plans in accordance with physician orders, payer requirements, and professional standards. Assess home environments and recommend equipment, modifications, safety strategies, or caregiver education as clinically appropriate. Communicate with the referring agency, physician, patient, family, PTA, and other care participants as permitted. Document patient progress accurately and efficiently to comply with payer, agency, and regulatory requirements.
