Physical Therapist (PT) in Home Health – contract independent contractor opportunities. Provide physical therapy services in patients’ homes across Southern California counties, evaluating patients, developing and implementing care plans to support mobility, safety, strength, balance, functional independence, and quality of life. Deliver skilled PT interventions in accordance with physician orders, payer requirements, and the approved plan of care; assess home environments and recommend equipment, safety strategies, or caregiver education; communicate with referring agency, physician, patient, family, PTA, and other care participants; document progress in digital workflow compliant with payer, agency, and regulatory requirements. Flexible schedule; set caseload and accept referral opportunities aligned with availability and service area. 100% digital administrative workflow with therapist-centered administrative support. New graduates and clinicians new to home health welcome. Areas with referral opportunities include Los Angeles, Riverside, San Bernardino, Ventura, Orange, and San Diego Counties (including Corona, Norco, Eastvale, Riverside, Chino Hills, Lake Elsinore, and surrounding communities). Requires California PT license, driver’s license, and professional liability insurance. Prioritizes independent contractor arrangements; opportunities for flexible schedules and varied coverage.
