Job Summary
The Physical Therapist will provide direct physical therapy services in home care settings based on the physician's plan of care. Responsibilities include assessing and evaluating patients' therapeutic and rehabilitative needs, developing comprehensive care plans, instructing patients and caregivers in therapeutic appliance use, and evaluating the home environment to support rehabilitation goals. The role requires a licensed physical therapist with a relevant degree and preferably one year of experience, particularly in home health.
Required Qualifications
- Licensed to practice as a physical therapist within the State
- Degree from a baccalaureate or master’s program in physical therapy approved by relevant associations
