Physical Therapist-Home Care Services

Provides in home physical therapy services to patients with neuro and/or musculoskeletal disorders under the direction of a physician. Responsibilities include independent scheduling of patient visits, weekend and on-call rotation, driving to patient homes, assessing and treating patients, documenting care, and coordinating with agency staff, physicians, hospital staff, and the public. Requires lifting up to 50 pounds, good communication and problem-solving skills, and the ability to work autonomously and handle emergencies; driving in hazardous conditions may be required.
