Job Summary
Provide in-home physical therapy services to patients with neuro and/or musculoskeletal disorders under physician direction. Independently schedule patient visits, manage daily office updates, and handle emergencies without assistance while working weekends and on-call rotations. Drive to hazardous conditions as needed, adhering to all safety protocols and map instructions. Maintain accurate documentation, communicate with agency staff, physicians, and the public, and manage confidential information. Requires current state licensure, CPR certification, and ability to lift up to 50 pounds frequently.
Required Qualifications
- Graduate of an accredited physical therapy program
- Current state license
- Valid drivers license
- CPR certification
- Good assessment and treatment planning skills
- Good communication and documentation skills
- Good time management and problem solving skills
- Good physical health
- Ability to lift up to 50 pounds frequently with occasional lifting of over 50 pounds
- Ability to work independently
- Ability to handle emergencies without assistance
- Ability to adapt to and work effectively with change
- Ability to take initiative
- Ability to set challenging objectives
- Ability to value and seek opportunity to learn
- Ability to drive in hazardous conditions

Desired Qualifications
- Two years experience in acute care or home care
