Physical Therapist - Home Care - PRN

Physical Therapist – Home Care (PRN) provides patient-centered rehabilitation services in patients’ homes, including comprehensive assessments, development of a Plan of Care for improved mobility and pain management, collaboration with clinical teams, and timely documentation. Requires Iowa licensure as a Physical Therapist, current BLS certification, mandatory reporter training, and a valid driver’s license; one year of experience preferred. Demonstrates strong interpersonal skills, effective written and verbal communication, and ability to work independently within Mercy Cedar Rapids Home Care context.
