Job Summary
Provide person-centered physical therapy to residents within our award-winning campus, eliminating travel time associated with traditional home care. Implement personal care plans that focus on residents' strengths and preferences while supporting their physical, spiritual, emotional, and clinical needs. Collaborate with the interdisciplinary clinical team to ensure comprehensive care is addressed throughout the continuum. Complete documentation to meet reimbursement and regulatory requirements, including OASIS, while maintaining current Maryland State Physical Therapy licensure. Required Qualifications
- Must meet current Medicare and state requirements
- Must possess a valid and current Maryland State Physical Therapy license
- Experience and the ability to complete documentation to meet reimbursement and regulatory requirements (ie: OASIS , etc.)
- Previous Home Care Physical Therapy experience required
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups is preferred
