Job Summary
Provide person-centered physical therapy support to residents at Riderwood Village by Erickson Senior Living, implementing personal care plans that focus on strengths, preferences, and routines. Deliver holistic care addressing physical, spiritual, emotional, and clinical needs while collaborating with the interdisciplinary clinical team to ensure resident needs are met throughout their continuum of care. Complete documentation to satisfy Medicare and state requirements, including OASIS, and maintain a valid Maryland State Physical Therapy license. Prior experience in home care is required, with therapeutic expertise in older adults across multiple diagnostic groups preferred.
Required Qualifications
- Must meet current Medicare and state requirements
- Must possess a valid and current Maryland State Physical Therapy license
- Experience and the ability to complete documentation to meet reimbursement and regulatory requirements (ie: OASIS , etc.)
- Previous Home Care Physical Therapy experience required
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups is preferred
