Job Summary
Provide person-centered physical therapy to residents within the award-winning Riderwood Village campus, eliminating travel time associated with traditional home care. Implement personal care plans that address physical, spiritual, emotional, and clinical needs while working collaboratively with the interdisciplinary team. Ensure all documentation meets Medicare and state regulatory requirements, including OASIS data collection, to support reimbursement. This role requires a Maryland-licensed Physical Therapist with specific home care experience, offering a $55/hour rate plus a $5,000 sign-on bonus for new hires.
Required Qualifications
- Must meet current Medicare and state requirements
- Must possess a valid and current Maryland State Physical Therapy license
- Experience and the ability to complete documentation to meet reimbursement and regulatory requirements (ie: OASIS , etc.)
- Previous Home Care Physical Therapy experience required
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups is preferred
