Job Summary
Provide person-centered physical therapy to residents in our award-winning campus, eliminating travel time for home care without the commute. Implement personal care plans that focus on residents' strengths, preferences, and routines while supporting their physical, spiritual, emotional, and clinical needs. Collaborate with the interdisciplinary clinical team to address resident requirements throughout their continuum of care. Complete documentation to meet reimbursement and regulatory standards, including OASIS. Requires a valid Maryland Physical Therapy license, current Medicare compliance, and specific home care experience.
Required Qualifications
- Must meet current Medicare and state requirements
- Must possess a valid and current Maryland State Physical Therapy license
- Experience and the ability to complete documentation to meet reimbursement and regulatory requirements (ie: OASIS , etc.)
- Previous Home Care Physical Therapy experience required
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups is preferred
