Job Summary
Provide person-centered physical therapy to residents at Riderwood Village by Erickson Senior Living, focusing on their physical, spiritual, emotional, and clinical needs. Implement personal care plans that prioritize residents' strengths, preferences, and routines while working with the interdisciplinary clinical team to address care throughout their continuum. Complete documentation to meet Medicare and state reimbursement requirements, specifically OASIS, and apply previous home care physical therapy experience to support older adults across multiple diagnostic groups.
Required Qualifications
- Must meet current Medicare and state requirements
- Must possess a valid and current Maryland State Physical Therapy license
- Experience and the ability to complete documentation to meet reimbursement and regulatory requirements (ie: OASIS , etc.)
- Previous Home Care Physical Therapy experience required
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups is preferred
