Job Summary
Provide person-centered physical therapy to residents, implementing personal care plans that focus on their strengths, preferences, and routines while ensuring comfort and companionship. Support the interdisciplinary clinical team to address physical, spiritual, emotional, and clinical needs throughout the continuum of care. Complete documentation to meet Medicare and regulatory reimbursement requirements, including OASIS, while maintaining a valid Maryland State Physical Therapy license. Prior experience in home care and therapeutic work with older adults across multiple diagnostic groups is required. This role offers flexibility within an award-winning campus, with compensation at $55.00 per hour plus a $5,000 sign-on bonus for direct hires.
Required Qualifications
- Must meet current Medicare and state requirements
- Must possess a valid and current Maryland State Physical Therapy license
- Experience and the ability to complete documentation to meet reimbursement and regulatory requirements (ie: OASIS , etc.)
- Previous Home Care Physical Therapy experience required
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups is preferred
