Job Summary
Provide person-centered physical therapy to residents at Riderwood Village by Erickson Senior Living, implementing personal care plans that focus on strengths, preferences, and routines while supporting physical, spiritual, emotional, and clinical needs. Collaborate with the interdisciplinary clinical team to address resident requirements throughout their continuum of care, specifically within our Certified Home Health team where residents live on-site, eliminating travel. Complete documentation to meet reimbursement and regulatory requirements, including OASIS, while maintaining a holistic approach to resident well-being. This role requires a valid Maryland State Physical Therapy license and previous Home Care Physical Therapy experience, with therapeutic expertise in older adults across multiple diagnostic groups preferred.
Required Qualifications
- Must meet current Medicare and state requirements
- Must possess a valid and current Maryland State Physical Therapy license
- Experience and the ability to complete documentation to meet reimbursement and regulatory requirements (ie: OASIS , etc.)
- Previous Home Care Physical Therapy experience required
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups is preferred
