Job Summary
Assess physical, psychological, social, and environmental needs of patients and families to develop tailored care plans under Licensed Prescriber orders. Integrate principles of development and disease process, including co-morbidities, while addressing deficits in strength, range of motion, flexibility, safety, and pain management. Provide skilled clinical care, patient education on self-care, and supervision of home health aides. Serve as a case manager coordinating with RNs, social workers, and other rehab disciplines, and refer vulnerable adults to Adult Protection Services as necessary. Maintain accurate records and anticipate unique community characteristics. Requires current Minnesota PT license, BLS certification, and reliable transportation for 2-3 days per week visits in the Delano, MN area.
Required Qualifications
- current license with the Minnesota Board of Physical Therapy
- Bachelors, Masters or Doctorate degree in Physical Therapy
- At least one year of previous experience as a PT in home care settings
- demonstrate knowledge of age related growth and development principles
- Proficiency with communication and data processing equipment, clinical apparatus related to home care, and electronic medical record system
- BLS certification through American Heart Association (AHA) or American Red Cross (ARC)
- reliable transportation for client visits
- work 2-3 days/week
- complete a total of 10 visits/week
