Job Summary
Deliver 1:1 physical therapy in patients’ homes as a Case Manager; evaluate patients and develop personalized treatment plans with a focus on mobility, fall prevention, and functional independence; assess home environments to promote safety; educate patients and caregivers on recovery strategies; collaborate with nurses, therapists, physicians, and social workers to support recovery; schedule autonomy in an in-home setting with travel to Chelsea, Winthrop, and East Boston areas; primarily serving Logan team patients and coordinating care across interdisciplinary team members.
Required Qualifications
- One year of experience in acute care, rehab or homecare as a Physical Therapist
- Registered with the Massachusetts Board of Registration in Physical Therapy
- Excellent verbal, written and electronic communications skills
- Computer proficiency and ability to learn new software
- Ability to prioritize work and adapt to changing situations
- Travel required within Mass General Brigham Healthcare at Home’s geographic area
