Job Summary
Provide physical therapy services in patients' homes with 1:1 care on the Ironworks team, delivering mobility-focused, safety-enhancing interventions. Develop personalized plans of care, educate patients and caregivers, and collaborate with nurses, therapists, physicians, and social workers to support independent living. Primary coverage area includes Lynn and Saugus, with Nahant support; schedule is part-time (32 hours) with a focus on high-quality, autonomous, home-based care and flexible planning.
Required Qualifications
- One year of experience in acute care, rehab or homecare as a Physical Therapist
- Registered with the Board of Registration in Massachusetts
- Excellent verbal, written and electronic communications skills
- Computer proficiency and ability to learn new software
- Ability to prioritize work and adapt to changing situations
- Travel required within Mass General Brigham Healthcare at Home’s geographic area
