Job Summary
Conduct in-home physical therapy evaluations and reassessments for patients to improve mobility, strength, balance, and safety. Create and progress individualized treatment plans while providing therapeutic exercises, gait training, and functional mobility training. Educate patients and caregivers on home exercise programs and safety strategies, then document visits and coordinate care with physicians and other healthcare providers. This role supports functional recovery within a network of over 500 outpatient clinics across the Northeast, Mid-Atlantic, Midwest, and Southeast.
Required Qualifications
- Licensed Physical Therapist
- Comfortable working independently
- Managing a mobile caseload
- Helping patients achieve safe functional outcomes in their home environment
