Job Summary
Conduct home care physical therapy evaluations, including interviews, observations, palpation, and specialized assessments like goniometry and muscle strength testing. Independently implement and modify treatment plans for patients with routine to complex problems, establishing teaching plans and visit schedules while communicating with patients, families, and the health team. Perform therapeutic exercises, functional mobility training, gait training, and modalities such as heat/cold application and ultrasound. Document interventions, maintain clinical records, and submit admission records within 48 hours and daily records within 24 hours with 95% accuracy. Organize patient assignments, participate in case conferences, and recommend adaptive equipment or home modifications. Requires a PT license in DC, MD, or VA, a Bachelor's degree, and 1-2 years of clinical experience.
Required Qualifications
- Bachelor's degree in Physical Therapy from an accredited school
- 1-2 years Clinical experience
- PT - Physical Therapist License to practice Physical Therapy in the District of Columbia Maryland or Virginia or any combination based on work location(s)
- DL - Valid State Driver's License in good standing with a dependable vehicle for transportation
- CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross
- Good clinical skills
- Organizational skills
- Excellent verbal and written communication skills
- Knowledge of home equipment home care reimbursement and regulations governing PTAs
