Job Summary
Conduct physical therapy evaluations, therapeutic exercise, gait training, and modalities for home care patients across Maryland, Virginia, and the District of Columbia. Independently implement and document plans of care, manage patient assignments, and coordinate discharge planning with physicians and community resources. Supervise physical therapist assistants and home health aides while maintaining clinical records with 95% audit accuracy. Perform home environment assessments to recommend adaptive equipment and durable medical equipment. Communicate regularly with patients, families, and the multidisciplinary team to revise goals and ensure continuity of care. Requires a PT license in DC, MD, or VA, a bachelor's degree, and 1-2 years of clinical experience.
Required Qualifications
- Bachelor's degree in Physical Therapy from an accredited school
- 1-2 years Clinical experience
- PT - Physical Therapist License to practice Physical Therapy in the District of Columbia Maryland or Virginia or any combination based on work location(s)
- DL - Valid State Driver's License in good standing with a dependable vehicle for transportation
- CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross
- Good clinical skills
- Organizational skills
- Excellent verbal and written communication skills
- Knowledge of home equipment home care reimbursement and regulations governing PTAs
- Verbal and written communication skills
- Basic computer skills preferred
- Smart Phone experience
