Job Summary
Physical Therapist Home Care role providing evaluation and physical therapy treatment in patients' homes under agency standards and regulatory requirements across MD, DC, or VA. Responsibilities include analyzing patient progress, revising plans of care, documenting interventions, establishing visit schedules, and communicating with patients, families, and health team members. Independently implements and documents plan of care for routine to complex cases, coordinates with case management, participates in case conferences, and maintains clinical records. Requires Bachelor’s degree in Physical Therapy, 1-2 years clinical experience, DC/MD/VA PT license(s) as applicable, valid driver’s license, and CPR/BLS/NRP certification. Skills include therapeutic exercise, gait and functional mobility training, home care reimbursement knowledge, and strong verbal/written communication. Supervises PTAs and home health aides as part of a multidisciplinary team; adheres to home care protocols and regulatory guidelines.
Required Qualifications
- Bachelor's degree in Physical Therapy from an accredited school required
- 1-2 years Clinical experience required
- PT - Physical Therapist License to practice Physical Therapy in the District of Columbia Maryland or Virginia or any combination based on work location(s)
- DL - Valid State Driver's License in good standing with a dependable vehicle for transportation
- CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross within 90 Days required
