Job Summary
Physical Therapist in Home Care providing evaluation and PT treatment across the DC/MD/VA region. Responsible for independently developing and implementing plans of care, documenting patient progress, and communicating with the health care team. Roles include evaluating patient needs, selecting appropriate interventions (therapeutic exercise, functional mobility training, gait training, heat/cold application, ultrasound), selecting adaptive equipment or home modifications, discharge planning, and preparing patient and caregiver education materials. Supervises PT assistants and maintains clinical records, with participation in case conferences and multidisciplinary quality improvement efforts. Requires strong clinical, organizational, and communication skills, familiarity with home care reimbursement and regulatory guidelines, and ability to coordinate care with physicians and other health care team members.
Required Qualifications
- Bachelor's degree in Physical Therapy from an accredited school required
- PT - Physical Therapist License to practice Physical Therapy in the District of Columbia, Maryland, or Virginia (or any combination based on work location(s))
- DL - Valid State Driver's License in good standing with a dependable vehicle for transportation (Upon Hire)
- CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from AHA or Red Cross within 90 Days (Upon Hire)
