Job Summary
Conduct physical therapy evaluations and treatment for patients in home care settings, including therapeutic exercise, gait training, and functional mobility training. Independently implement and modify plans of care based on patient progress, while documenting interventions, assessments, and teaching plans for patients and families. Manage patient assignments, maintain clinical records with high accuracy, and coordinate with multidisciplinary teams and community resources to support discharge planning and goal achievement. Perform history and assessments to determine rehabilitation needs, recommending adaptive equipment and home modifications as necessary.
Required Qualifications
- Bachelor's degree in Physical Therapy from an accredited school
- 1-2 years Clinical experience
- PT - Physical Therapist License to practice Physical Therapy in the District of Columbia Maryland or Virginia or any combination based on work location(s)
- DL - Valid State Driver's License in good standing with a dependable vehicle for transportation
- CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross
- Good clinical skills
- Organizational skills
- Excellent verbal and written communication skills
- Knowledge of home equipment home care reimbursement and regulations governing PTAs
