Job Summary
Physical Therapist position at Holton Manor in Elkhorn, WI (on-site). Flexible scheduling (Full-Time, Part-Time, PRN) with days, evenings, and Saturdays. Provide skilled therapy services across inpatient, outpatient, and home health programs, collaborating with a team of therapists. Responsibilities include initial patient examinations, developing and revising plans of care, maintaining patient records, coordinating discharge plans with the PTA, ensuring compliance with practice standards and payer documentation, and mentoring/overseeing PTA-delivered care. Requires Wisconsin physical therapy license and graduation from an accredited PT program; strong administrative, documentation, and multidisciplinary collaboration skills. Required Qualifications
- Graduate of an accredited education course in physical therapy
- Licensed in the state of Wisconsin
- Must not be excluded or sanctioned from state or federal program
- Demonstrate professional attitude and appearance
- Proficient administrative and communication within department and facility and company
- Thorough knowledge of the principles, methods, materials, and equipment used in the practice of their discipline
- Display working knowledge of multidisciplinary practice scopes in the Outpatient, ALF, ILF or LTC setting
- Thorough knowledge of all payer source documentation and payment requirements
- Ability to use analytical skills to develop outcome driven solutions
- Prior EMR/computer experience
- Must meet all health, background check, and training requirements as required by Wisconsin law
