Physical Therapist - Holbrook, NY ($5k BONUS!!)

Initiate, review, and implement the physical therapy plan of care for each patient based on initial assessments and prescribed guidelines. Educate patients and caregivers regarding disease processes, medications, and individualized treatment plans. Maintain and process all documentation in compliance with agency standards using the electronic medical record system, submitting clinical records and time logs within required timeframes. This field-based role covers the Greater Holbrook area and requires a licensed Physical Therapist with at least one to two years of clinical experience. Candidates must have reliable transportation and basic knowledge of the iOS operating system. A productivity bonus structure and comprehensive benefits package are included.
