Required Qualifications

• •Bachelor's degree in Physical Therapy
• •PT Licensure from the Board of PT Examiners or PT Compact Commission
• •Advanced level of clinical and interpersonal skills
• •Advanced knowledge of diagnoses/conditions seen in physical therapy
• •Advanced working knowledge of various Physical Therapy techniques
• •Sound judgment in delegating task to Physical Therapy Assistants and Rehab Techs
• •Sound judgment in selecting the proper mode of communication which conveys a favorable image of the therapy department and Prisma Health
• •Good judgement in selecting the proper mode of communication
• •Knowledgeable and compliant with Prisma Health's purpose
• •Compliance with laws and regulations of appropriate state and federal agencies and Joint Commission guidelines
• •Compliance with department policies and procedure
• •Demonstrates awareness and use of proper safety measures regarding the patients, others in the treatment area and self
• •Respectful of the rights of other team members
• •Ability to perform appropriate initial physical therapy evaluations and re-evaluations
• •Ability to design a physical therapy plan of care
• •Ability to perform and interpret standardized evaluations appropriate for the patient
• •Ability to identify actual or potential problems that would impact the PT treatment plan
• •Ability to establish specific treatment goals based on the results of the initial assessment
• •Ability to design a plan of intervention that is coordinated and integrated with other services
• •Ability to revise the treatment plan as indicated
• •Ability to implement and manage a specific and comprehensive treatment plan
• •Ability to participate in patient discharge planning
• •Ability to demonstrate flexibility and creativity in the implementation of therapy techniques
• •Ability to interact regularly with others involved in the patient's care to incorporate an interdisciplinary treatment approach
• •Ability to reassess patient status in response to treatment and goal achievement
• •Ability to redefine problems and modify goals and treatments as indicated
• •Ability to provide information regarding discharge and follow-up services
• •Ability to provide education and training to patients, families, and caregivers regarding diagnoses, progress, and long-term functional outcomes
• •Ability to complete the Plan of Care and Patient/Family Education Documentation
• •Ability to teach, counsel, and communicate with the patient/family/caregiver educational information related to the patient's diagnosis/condition
• •Ability to provide educational material to patients/families as indicated
• •Ability to document all aspects of assessment, planning, treatment, and discharge in the medical record in a legible and timely manner
• •Ability to ensure goals are appropriate and measurable within a specified time frame
• •Ability to ensure treatment notes are clear, succinct, and contain information regarding treatment progression
• •Ability to utilize appropriate abbreviations and medical terminology
• •Ability to attend staff meetings, team meetings, and mandatory in-services as scheduled
• •Ability to initiate and manage assignments in a timely and thorough manner
• •Ability to assist with care and maintenance of departmental equipment and supplies
• •Ability to complete charge/productivity documentation and enter charges in an accurate and timely manner
• •Ability to demonstrate willingness to assist
• •Ability to provide supervision/mentorship to Physical Therapists, Physical Therapy Assistants and Rehab Aides
• •Ability to provide feedback on performance in a professional, tactful and beneficial manner
• •Ability to provide supervision to Physical Therapy students as assigned
• •Ability to participate in student orientation and provide effective learning experiences, feedback and timely evaluation
• •Ability to meet the team/department productivity standards
• •Ability to comply with the established standards of efficient scheduling
• •Ability to establish annual goals related to stated corporate and department initiatives
• •Ability to provide supporting information related to the progress made toward achieving established annual goals
• •Ability to support corporate leadership initiatives
