Job Summary
Provide one-on-one physical therapy to older adults in private residences or senior living communities, perform evaluations, develop individualized plans of care, and deliver evidence-based treatment to improve mobility, independence, and aging-in-place. Document effectively in FOX’s EMR, collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists), educate patients, caregivers, and staff, and offer telehealth/virtual care when appropriate to extend access and flexibility. The role supports in-home and senior living settings with salaried Full-time, Part-time, and PRN pathways; H1B sponsorship is available for qualified clinicians; new graduates may join the Emerging Professionals Mentor Program.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
