Job Summary
Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician. Evaluate patient needs to develop rehabilitation goals that improve mobility, strength, balance, and overall physical function. Adjust treatment plans and techniques as needed to achieve optimal outcomes while collaborating with the interdisciplinary home health team. Educate patients and caregivers on exercises, safety, and mobility strategies, and document all visits, assessments, and progress notes accurately. Support patients in maximizing their independence and improving quality of life within home environments.
Required Qualifications
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- CPR certification
- Strong communication, assessment, and problem-solving skills
- Ability to work independently in a variety of home environments
- Valid driver's license, auto insurance, and reliable transportation
- This is a safety-sensitive position
- Employee must meet minimum requirements to be eligible for benefits
- Employee must meet state specific requirements

Desired Qualifications
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
- In-home therapy delivery
- Geriatric rehab, orthopedic or neurologic rehab
- Post-acute care or chronic condition management
- Working as part of a mobile or interdisciplinary care team
