Job Summary
Provide primary care to 25-35 patients in a home setting, developing and revising plans of care to meet physical, emotional, and developmental needs. Coordinate all rehabilitative disciplines, supervise PTAs and Home Health Aides, and adjust visit frequency based on physician orders and clinical status. Manage the patient journey from admission through recertification and discharge, while communicating status changes, lab results, and care team updates to the physician. Complete required OASIS documentation, participate in performance improvement activities, and attend continuing education to maintain clinical knowledge. Weekend and call rotation as required.
Required Qualifications
- Graduate of an accredited school of physical therapy
- Minimum 1 year of Full-time Physical Therapy experience
- Strong critical care assessment and rehabilitation skills
- Use of Windows based computer
- Utilizing home health patient software
- Email proficiency
- Internet proficiency
- Computer based learning modules proficiency
- Good understanding of home health operations
- Good understanding of reimbursement
- Good understanding of law
- Good understanding of regulatory processes
- Weekend and call rotation as required
