Physical therapist - health - therapy

Plan and administer prescribed physical therapy treatment for patients in their homes to attain the highest level of physical function. Assess and document patients' physical strengths to support physician evaluations and guide treatment goals, while developing and implementing rehabilitation programs consistent with the physician's Plan of Treatment. Provide clear written care instructions to PTAs and Home Health Aides, overseeing their performance and documentation. Report patient progress to the physician, clinical manager, care team, and family, submitting all required evaluations, plans, and discharge summaries. This role requires a current and unrestricted Physical Therapy license, one year of experience, and a valid state driver's license. Weekend coverage from Friday through Monday is required for full-time employment.
