Job Summary
Evaluates and conducts medically prescribed physical therapy treatment programs to treat movement dysfunction and pain, including initial and ongoing assessments, plan of care development, patient/family education, discharge planning, and coordination with care teams. Establishes and revises a plan of care, conducts therapy interventions using standard PT techniques, provides education to patients and families, supports continuity of care through discharge planning and home modifications, and documents patient responses to therapy. Supervises physical therapy assistants, students, and other support staff as appropriate and ensures regulatory-compliant documentation. Requires a Bachelor's, Master's, or Doctorate in Physical Therapy and active licensure in the practice states; BLS HCP certification within 90 days of employment.
Required Qualifications
- Licensed Physical Therapist in IL, MO, OK, and WI as applicable per state of work location
- State licensure in good standing (IDFPR/OK Medical Board/Wisconsin DSPS/Missouri DPR)
- Basic Life Support Healthcare Provider (BLS HCP) certification within 90 days of employment
