Job Summary
Conduct one-on-one physical therapy for older adults in private residences and senior living communities, including memory care and independent living settings. Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment to improve mobility, independence, and quality of life. Document sessions within FOX's EMR system and collaborate with interdisciplinary teams of PTs, OTs, SLPs, and Fitness Specialists. Educate patients, caregivers, and staff on safety and aging in place, while incorporating telehealth and virtual care when appropriate to extend access. Manage your own treatment schedule with flexibility to create and adjust sessions as needed.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
- Ability to exert medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
- Ability to perform CPR
- Ability to provide moderate to maximum assistance with patient transfers and bed mobility
- Ability to reposition patients, stand/walk for extended periods, climb stairs, and perform therapeutic activities
- Ability to drive to patient homes and senior living communities as part of the Geriatric House Calls model
- Ability to wear PPE when required
- Must be available for weekend shifts
- Must be available for travel to patient homes and senior living communities
Desired Qualifications
- Experience with telehealth and other forms of virtual care
