Job Summary
Provide one-on-one physical therapy to older adults in their homes or within senior living communities, including private residences and memory/assisted living settings. Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment to improve mobility, independence, and safety. Deliver therapy sessions that support wellness and functional independence; document effectively in FOX’s EMR; collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists); educate patients, caregivers, and staff to promote aging-in-place; optionally incorporate telehealth/virtual care to extend access and flexibility. Requires active PT license in practice state(s), degree from an accredited PT program, CPR certification, and proficiency with EMR systems. H1B sponsorship is available for qualified clinicians; new graduates are eligible for FOX’s Emerging Professionals Mentor Program.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
