Job Summary
Provide one-on-one physical therapy to older adults in private residences or senior living communities, perform evaluations and develop individualized plans of care, deliver evidence-based treatment to support mobility and independence, document in FOX’s EMR per regulatory standards, collaborate with an interdisciplinary team (PT/OT/SLP/Fitness Specialists), educate patients, caregivers, and staff to promote safety and aging in place, and offer telehealth/virtual care when appropriate. Roles available include Full-time In-Home & Senior Living Care, Part-time In-Home & Senior Living Care, and PRN/Fox Flex Home & Community Visits, with H1B sponsorship available and the Emerging Professionals Mentor Program for new graduates.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice
- Degree from an accredited Physical Therapy program
- Current CPR certification
- Basic computer and EMR proficiency
