Job Summary
Provide one-on-one physical therapy to older adults in private residences, senior living communities, and other home-based settings under FOX Rehabilitation's Geriatric House Calls model. Conduct evaluations, develop individualized care plans, and deliver evidence-based therapy to improve mobility, independence, and quality of life. Perform and document therapy sessions within the EMR, collaborate with an interdisciplinary PT/OT/SLP team, and educate patients, caregivers, and staff to promote safety and aging in place. Telehealth/virtual care may be incorporated to extend access and flexibility. Roles available include in-home and senior living care (full-time, part-time, and PRN) in Lee's Summit, MO and surrounding communities. Required Qualifications
- Licensed Physical Therapist in MO
- Degree from an accredited Physical Therapy program
- Current CPR certification
- Proficiency with EMR
- Ability to perform in-home visits and work in senior living/community settings
- Eligible for H1B sponsorship (optional)
