Job Summary
Provide one-on-one physical therapy to older adults in private residences and senior living communities across Columbus, IN, delivering evaluations, individualized plans of care, and evidence-based treatment to improve mobility, independence, and quality of life. Document patient progress in FOX’s EMR, collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists), and educate patients, caregivers, and staff to promote aging-in-place safety. Telehealth/virtual care may be incorporated to extend access. Flexible scheduling and autonomy to manage treatment plans and patient relationships within in-home and senior living settings; opportunities across Full-time, Part-time, and PRN/flex roles, with H1B sponsorship available for qualified clinicians.
Required Qualifications
- Valid Physical Therapy license in the state of practice
- Degree from an accredited Physical Therapy program
- Current CPR certification
- EMR proficiency or experience with electronic medical records
- Ability to drive to patient homes and locations within Columbus, IN area
