Job Summary
Provide one-on-one physical therapy to older adults in-home or within senior living communities (private residences, independent living, assisted living, memory care). Develop individualized plans of care, deliver evidence-based treatment, and document in FOX’s EMR system. Deliver therapy sessions to improve mobility, independence, and safety with aging in place. Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists) and educate patients, caregivers, and staff. Telehealth/virtual care optional to extend access and flexibility; manage own treatment schedule across in-home and senior-living settings.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
