Job Summary
Provide one-on-one physical therapy to older adults in private residences and senior living communities across Greater Boston. Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment sessions supporting mobility, independence, and quality of life. Document effectively within the EMR system and collaborate with an interdisciplinary team including OTs, SLPs, and fitness specialists. Educate patients, caregivers, and staff on safety and aging in place while incorporating telehealth and virtual care when appropriate. Drive to patient locations as part of the Geriatric House Calls model, adhering to physical demands for transfers and patient assistance.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
- Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
- Performing CPR
- Providing moderate to maximum assistance with patient transfers and bed mobility
- Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
- Driving to patient homes and senior living communities as part of the Geriatric House CallsTM model
- Wearing PPE when required
