Job Summary
Provide one-on-one physical therapy to older adults in private residences, senior living communities, and other home settings under FOX Rehabilitation's Geriatric House Calls model. Responsibilities include performing evaluations, developing individualized plans of care, delivering evidence-based in-home therapy to improve mobility and functional independence, documenting in FOX’s EMR, collaborating with PT/OT/SLP and Fitness Specialists, educating patients, caregivers, and staff to promote safety and aging-in-place, and offering telehealth/virtual care when appropriate; opportunities to work in in-home and senior living settings with flexible schedules and a clinician-driven culture.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
