Job Summary
Physical Therapist in Bloomington, IN with FOX Rehabilitation delivering one-on-one, in-home and senior living care to older adults. Responsibilities include evaluating and treating patients across private residences and senior living communities, developing individualized plans of care, documenting in FOX’s EMR, collaborating with PT/OT/SLP and staff, educating patients and caregivers, and delivering telehealth/virtual care when appropriate. Opportunities to work full-time, part-time, or PRN; H1B sponsorship available. Emphasis on autonomy, flexible schedules, aging-in-place goals, and interdisciplinary teamwork. Roles involve in-home visits and may include senior living settings with optional telehealth components. Commitment to mobility, safety, and quality of life through evidence-based treatment and patient-centered care.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Current CPR certification
- Basic computer and EMR proficiency
