Job Summary
We’re looking to add to our extraordinary care team. Grounded by our belief that outstanding care is best delivered in a team-based environment, our home health physical therapists collaborate to provide management and delivery of patient care plans. This position supports patients in Jackson and surrounding areas. Responsibilities include providing Physical Therapy services in home-health settings, using EMR software (e.g., TruChart EHR), driving locally to patient homes (~20%), maintaining timely charting, and coordinating with clinical educators, preceptors, and supervisors. Must hold a Bachelor’s or Master’s degree in Physical Therapy, be a Licensed Physical Therapist in MI, and have 1+ year healthcare experience with EMR. Schedule is Monday-Friday, 8:00a/9:00a-5:00p with local travel to residential homes; includes flexible scheduling benefits and a supportive environment with technology to streamline charting.
Required Qualifications
- Bachelor's degree or higher
- Healthcare (1+ years)
- 1+ years of experience in EMR (Electronic Medical Record)
- TruChart EHR (or Other EMR)
- Valid Regular Driver License (DL)
- Valid Physical Therapist license in Michigan (MI)
- Local travel to patient locations (~20% of day)
- Ability to spend ~20% of your day driving to/from patient locations
- Commitment to charting deadlines
- Experience with tablets, mobile devices and EMR software
