Job Summary
Provide one-on-one physical therapy to older adults in private residences and senior living communities, developing and delivering individualized treatment plans to improve mobility, independence, and quality of life. Conduct evaluations, create plans of care, document in the EMR, collaborate with an interdisciplinary team, and educate patients, caregivers, and staff to promote safety and aging in place. Telehealth/virtual care may be incorporated to extend access. Must hold a valid Physical Therapy license and CPR certification; opportunities span in-home, senior living settings, and optional telehealth components.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
