Physical Therapist

Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician. Evaluate patient needs to improve mobility, strength, balance, and overall physical function, then adjust treatment plans as needed to achieve optimal outcomes. Collaborate with the interdisciplinary home health team—including nurses, occupational therapists, and social workers—to provide coordinated care. Educate patients and caregivers on exercises, safety strategies, and assistive device use while documenting all visits, assessments, and progress notes accurately. Work Monday through Friday from 8am to 5pm in Ft. Wayne, IN, with a Master's degree in Physical Therapy and current state licensure required.
