Job Summary
Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed with the referring physician. Evaluate patient needs to set rehabilitation goals and adjust techniques to improve mobility, strength, and balance. Collaborate with the interdisciplinary home health team, including nurses and occupational therapists, to provide coordinated care. Educate patients and caregivers on exercises, safety strategies, and assistive device use. Document all visits, assessments, and progress notes accurately. Work Monday through Friday from 8am to 5pm in Ft. Wayne, IN, with a Master's degree and current Indiana PT licensure required.
Required Qualifications
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- CPR certification
- Strong communication, assessment, and problem-solving skills
- Ability to work independently in a variety of home environments
- Valid driver's license, auto insurance, and reliable transportation
- Monday-Friday, 8am-5pm schedule
- Ft. Wayne, IN location
Desired Qualifications
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
- In-home therapy delivery
- Geriatric rehab, orthopedic or neurologic rehab
- Post-acute care or chronic condition management
- Working as part of a mobile or interdisciplinary care team
