Job Summary
Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician. Evaluate patient needs to improve mobility, strength, balance, and overall function while adjusting treatment plans as needed. Collaborate with the interdisciplinary home health team including nurses, occupational therapists, and social workers to provide coordinated care. Educate patients and caregivers on exercises, safety strategies, and assistive device usage. Document all visits, assessments, and progress notes accurately and in a timely manner. Support patients in maximizing their independence and improving quality of life within their homes.
Required Qualifications
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- CPR certification
- Strong communication, assessment, and problem-solving skills
- Ability to work independently in a variety of home environments
- Valid driver's license, auto insurance, and reliable transportation
- Monday-Friday, 8am-5pm schedule
- Ft. Wayne, IN location
Desired Qualifications
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
- In-home therapy delivery
- Geriatric rehab, orthopedic or neurologic rehab
- Post-acute care or chronic condition management
- Working as part of a mobile or interdisciplinary care team
