Job Summary
Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician. Evaluate patient needs, adjust treatment plans, and collaborate with interdisciplinary home health teams to improve mobility and function. Educate patients and caregivers on exercises, safety strategies, and assistive device usage while documenting all visits and progress notes accurately. This role requires a Michigan-licensed Physical Therapist with a Master's degree, CPR certification, and valid driver's license to work independently in home environments. Full-time coverage in Saginaw territory includes a $7,500 sign-on bonus, fleet car program, and comprehensive benefits. Required Qualifications
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- CPR certification
- Strong communication, assessment, and problem-solving skills
- Ability to work independently in a variety of home environments
- Valid driver's license, auto insurance, and reliable transportation
Desired Qualifications
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
- In-home therapy delivery
- Geriatric rehab, orthopedic or neurologic rehab
- Post-acute care or chronic condition management
- Working as part of a mobile or interdisciplinary care team
