Job Summary
Deliver skilled physical therapy to patients based on individualized Plans of Treatment developed in coordination with referring physicians. Evaluate patient needs, adjust treatment plans, and educate caregivers on mobility strategies and assistive devices. Document all visits, assessments, and progress notes accurately. Collaborate with interdisciplinary home health teams including nurses and occupational therapists to provide coordinated care. Maintain a Monday-Friday, 8am-5pm schedule with full benefits, 401k match, and a $5,000 sign-on bonus. Requires a Master's degree in Physical Therapy, current Indiana licensure, and CPR certification.
Required Qualifications
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- CPR certification
- Strong communication, assessment, and problem-solving skills
- Ability to work independently in a variety of home environments
- Valid driver's license, auto insurance, and reliable transportation
- Monday-Friday, 8am-5pm schedule
- Ft. Wayne, IN location
Desired Qualifications
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
- In-home therapy delivery
- Geriatric rehab, orthopedic or neurologic rehab
- Post-acute care or chronic condition management
- Working as part of a mobile or interdisciplinary care team
