Job Summary
Full-time Physical Therapist needed for home-health services in Ft. Wayne, IN. Deliver skilled physical therapy to patients at home, develop plans of treatment with the referring physician, evaluate and adjust rehabilitation goals to improve mobility, strength, balance, and overall function. Collaborate with an interdisciplinary home health team (nurses, occupational therapists, speech therapists, social workers), educate patients and caregivers on exercises, safety, mobility strategies, and assistive devices, and document visits and progress. Requires IN licensure, CPR, and ability to work independently; driver’s license and reliable transportation required; schedule is Monday–Friday, 8am–5pm, with potential sign-on bonus and benefits.
Required Qualifications
- Master’s degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- CPR certification required
- Strong communication, assessment, and problem-solving skills
- Ability to work independently in a variety of home environments
- Valid driver’s license, auto insurance, and reliable transportation
