Job Summary
Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician. Evaluate patient needs, adjust treatment plans, and educate families on exercises and safety strategies to improve mobility and independence. Document all visits, assessments, and progress notes accurately while collaborating with the interdisciplinary home health team. Requires a Michigan PT license, Master's degree, and CPR certification. Full-time role in Midland, MI with a $7,500 sign-on bonus, fleet car option, and comprehensive benefits including medical, dental, vision, and 401k match.
Required Qualifications
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- CPR certification
- Strong communication, assessment, and problem-solving skills
- Ability to work independently in a variety of home environments
- Valid driver's license, auto insurance, and reliable transportation
- PT Licensure - Michigan

- Adjust treatment plans and techniques as needed to achieve optimal outcomes
- Collaborate with the interdisciplinary home health team—including nurses, occupational therapists, speech therapists, and social workers—to provide coordinated care
- Educate patients and caregivers on exercises, safety, mobility strategies, and use of assistive devices
- Document all visits, assessments, and progress notes accurately and in a timely manner
- Support patients in maximizing their independence and improving quality of life—wherever they call home
Desired Qualifications
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
- In-home therapy delivery
- Geriatric rehab, orthopedic or neurologic rehab
- Post-acute care or chronic condition management
- Working as part of a mobile or interdisciplinary care team
